Stay healthy, remain independent, and age in place in your community with a free customized whole-person evaluation for older adults age 60 years and older. We assess your needs and then provide referrals for services including housing, nutrition, access to health care, social activities, and transportation.

Our goal is to support older adults to find the right services so that they can thrive.

We invite you to choose the design and delivery of services that best support the life you want to live.

We Assist You With Referrals For:

  • Housing resources.
  • Nutrition, including healthy meals.
  • Access to health care including transportation for medical appointments, navigating the health care system, health insurance, preventative care, and medication review.
  • Staying connected and reducing isolation with caregiver support & respite, companionship, and volunteer opportunities.
  • Transportation for grocery shopping and running errands.

This service is available in Clay, Otter Tail, Traverse and Wilkin counties in Minnesota and Cass and Ransom counties in North Dakota.
