If you are a parent or caregiver to a youth with a disability who is about to turn 18, supported decision-making can build a support network around your youth to help them to independence.
It is a model of care designed to help individuals avoid unnecessary guardianship.
A supported decision-making plan promotes independence and provides support in navigating everyday life decisions, such as where to live, employment, social interactions, health care and more. It recognizes that everyone has the right to make decisions about their own lives, even when they need support.
LSS Supported Decision-Making services can help individuals at risk of guardianship explore paths to remaining independent and can also be a path to restoration of rights for some individuals already under guardianship.
