It’s available to individuals of any age who may need long-term care or assistance to live independently, such as individuals with disabilities and older adults.
How It Works
It is a free, in-person assessment that includes an interview with a trained assessor who will ask you questions about your activities of daily living, where you want to live, how you want to participate in the community, your medical history, and any other needs you have in order to create a personalized care plan.

In order to access programs funded by Medical Assistance (Minnesota’s Medicaid program), you must first take a MnCHOICES Assessment. These programs include Medical Assistance waiver programs and Community First Services and Supports (CFSS).
How To Get Started
To get started, contact your county or tribal human services agency or Managed Care Organization (MCO) to ask for a MnCHOICES assessment. The agency can provide information about the available programs, eligibility requirements and how to apply.
How To Prepare

- Think about any challenges or barriers you have in doing everyday activities and things you enjoy.
- Invite any family, friends or others who support you to attend your assessment.
- Have a list of your medications.
What Happens After the Assessment
After the assessment, the assessor will summarize what they learned about you and your needs. You will receive a summary of the assessment along with information about the programs or services you qualify for. From there, you can choose your providers and develop your care plan. Once your plan is approved and all the paperwork is completed, you can begin receiving services.
