Coordinate services to ensure proper case management, making informed decisions regarding proper living arrangements and medical needs. Maintain client files, document case work, and manage client assets. Incumbent will be on-call on a rotating basis.
BA degree in Human Services or other relevant field, or four years experience in a closely related field, with similar clients and decision making. In addition to the experience equivalent to the degree, two years experience working with similar client needs served by the program is required. Knowledge of community resources, legal, and medical services would be beneficial. Valid driver’s license and bonding eligibility.
